Agency Application

If you wish to refer a young person for supported accommodation at Park Lodge Project please complete the Agency Application Form. As part of the Agency Application Form you are also requested to complete our standard reference form.
In addition, the young person must complete the Young Person’s Application Form.

When all the forms have been completed please return them to Park Lodge Project. This can be done by email to: adminclerk@parklodgeproject.org or by fax: 0116 2709326 or by post or hand delivered to Park Lodge Project, 2 Central Avenue, Leicester LE2 1TB.
The application cannot be processed without all the enclosed information being completed and returned to us.

Young Persons Application

As a young single person aged 16 –25 years who is homeless or threatened with homeless you may apply to Park Lodge Project for supported accommodation. Please complete the form and return to us as quickly as possible ensuring that you complete the form accurately and provide two referees.
